To register for the cruise simply print and complete the second page and mail or FAX to Group Cruises Unlimited using the instructions on the bottom of the form. Be sure to fill in the Sponsor block so that we may give proper recognition to the sponsoring organization.
If you have cruised with us before and you want to use the same credit card we have on file just put in the last four digits of the card instead of all the information.
Alternatively, you may use this form to enter the registration information using your computer. You then have three submission options:
1. Click the red button on the form and submit via email;
2. Print the form and submit via FAX; or
3. Print the form and submit via mail.
Please note that your FAX will be sent as an email attachment. If you are sending your credit card information via FAX or email DO NOT ENTER YOUR COMPLETE CREDIT CARD NUMBER. Just fill in the first four digits and the last four like this: 1234XXXXXXX4321 or leave the credit card number blank. Group Cruises Unlimited will call you to obtain the missing info.
